To schedule a visit with Dr. Kachmann in the Holistic Pain Management Clinic, call 260-436-2416 ext 2108. Patients are seen by appointment only. All clinic visits are scheduled at Lutheran Hospital Cancer Resource Center Clinic.

To schedule a private session with one of our Therapists, please call 260-420-YOGA (9642).

For your first visit, please bring the following items:

• Yoga Therapy Questionnaire - mailed prior to initial therapy session
• Massage Therapy Questionnaire - Mailed Prior to initial therapy session
• Insurance card
• Referral from your care physician (if needed)
• Results of diagnostics testing (MRIs, CAT scans, etc.)
• List of prescriptions and medical conditions
• Medical records
